TMJ Solutions Website Launch

Whether it's awareness or patient education, I've noticed an increase in the number of patients asking me about TMJ / TMD disorders and how it can be fixed. The symptoms, which may include a number of ailments, are often misdiagnosed as musculoskeletal or neurological disorders and first attempts at treatment usually comes in the form of prescription medication or even surgery. While it may be easy to treat the acute symptoms first, not solving the root cause will simply compound into a cascade of symptoms down the road.

When it comes to TMJ / TMD, I encourage people to do your research first, and the internet of course is a great tool to do so. The Mayo Clinic, TMJ Association, and LVI provides some great information, as well as patient forums who can share their TMJ stories with you.

In response, I have launched a simple website called www.tmjsolution.com, which explains what treatment options I can provide. I have provided some helpful information to get you started, and links to external resources for your own diligence.

If you suspect symptoms such as jaw pain, migraines, or vertigo may be related to TMJ, give us a call and I can check out the problem.

Under Armour Video - George St. Pierre - MMA Mouthguard

A few weeks ago, I made an Under Armour mouthguard for my husband, Mike, who has been a long time practitioner of Shuai-jiao (Chinese martial art). On more than one occasion he's come home with a few chipped teeth because 1.) He has no common sense and doesn't always wear a mouthguard, and 2.) He has a bad habit of always clenching his teeth, which eventually grinds them down to a point where they occasionally fracture.

So to see if this technology works for the average weekend warrior, who makes for a better guinea pig than my husband right?

Here's his feedback

"Okay, I'm going to first start out and tell you what I hate about it...I hate the fact that I can't say anything with this thing on. In (Under Armour's) defense, I hate the feel of all mouthguard s anyways, so this is no different. Otherwise, I was actually very pleased at the immediate improvement I felt when it came to head and neck movement, and overall breathing. Shuai-jiao is all throws, so when you're going for a throw, head movement is important to get the proper form and leverage when executing. By opening up and repositioning my jaw, I'm able to that extra range of motion in my neck and shoulder area. Breathing is where I felt the best improvement. I never realized that when I naturally clench my teeth during my workouts, how much I'm obstructing my airway. Opening up my jaw position immediately improved airflow, and I didn't burn out as quickly. I know it sounds crazy that my new mouthpiece made such a difference, but psychosomatic or not, it worked."

A Device to De-Stress Your Workout

AFTER a night that included several beers and not enough sleep, Keith Gillis, a 31-year-old cyclist in Truro, Nova Scotia, set out on a 74-mile road ride with the caveat that he was feeling fatigued. Yet two-thirds of the way through the ride, Mr. Gillis said, he was setting the pace, riding ahead of his cycling partner. “Even though I’d felt tired at the start, I had the energy to lead, and I wasn’t out of breath,” he said.

To what did he attribute his stamina? A flexible mouth guard by a Canadian company called Makkar that he has been biting on while riding since April. When fellow cyclists ask him why he isn’t winded when they are, Mr. Gillis tells them, “because I have my Superman guard in.”

Mr. Gillis is among a small but growing number of athletes wearing what manufacturers like to call “performance mouthpieces” while cycling, running or weight training. One of the newest tools in a performance-enhancement arsenal, these mouthpieces are light, flexible pieces of molded plastic that fit over the teeth — and are only vaguely reminiscent of that retainer from junior high school or the bulky mouth guards worn by football players.

Dentists say these high-end mouth guards can open up the airways, prevent teeth-clenching and align the jaw. Being able to take in more air while exercising has obvious benefits — more oxygen for working muscles — while a relaxed jaw can decrease stress and help an athlete’s body function more efficiently.

“There is research to support improved breathing mechanics and reduced jaw fatigue,” said Fabio Comana, an exercise physiologist with the American Council on Exercise. “Depending on how you look at it, there is some truth to the claims.”

Unlike regular mouth guards, which are available off the shelf and at modest prices, performance mouthpieces cost hundreds or even thousands of dollars and must be custom-fitted by a dentist. Ordinary mouth guards are usually dropped in boiling water and fitted to the wearer’s teeth to protect against injury. With performance mouth guards, the idea is to reposition the jaw, anywhere from a few millimeters to, in the case of Mr. Gillis, a quarter of an inch sideways.

Two main brands are on the market — Makkar and Under Armour — and each makes the claim that it can increase an athlete’s strength, reduce stress and improve overall performance. Professional athletes have taken note: during the World Series this year, television viewers could see Derek Jeter wearing a mouthpiece made by Bite Tech, the company that developed the technology that Under Armour uses. In early November, Jon Gruden of ESPNsaid on “Monday Night Football” that many of the New Orleans Saints wore Makkar mouth guards.

The Makkar Pure Power Mouthguard (or PPM, as the company calls it) was introduced in 2006 and costs from $595 to $2,250, not counting the dentist’s fee. Makkar’s Web site touts the mouth guard’s use in diverse sports, including golf, soccer, swimming and tennis, and includes endorsements by the basketball player Shaquille O’Neal and the football player Terrell Owens.

Under Armour’s line of Performance Mouthwear was introduced in September with a basic price of $495. Among the professionals who wear them are Adrian Peterson of the Minnesota Vikings and Marian Gaborik of New York Rangers.

There is a big difference between the two brands: While the Makkar product must be gripped between the upper and lower teeth, the Under Armour one sits only on the lower teeth. But both are meant to set the jaw at ease.

“When you have the jaw in relaxed position, the rest of the body can be more relaxed — it’s a domino effect,” said Kathrina Agatep, a dentist in San Diego who sells both brands.

Repositioning the jaw is not the same as keeping the jaw slack while exercising. “Even if you have your mouth open when you run, that doesn’t necessarily mean the joint and rest of your body is in the maximal alignment,” Dr. Agatep said.

While the products’ potential benefits may sound good, it isn’t clear how much of an edge they actually confer. A study sponsored by Makkar in 2008 at Rutgers University found that athletes wearing Pure Power Mouthguards could jump higher and perform better at their peak, but it did not find that their endurance was any better.

“There wasn’t a huge difference,” said Shawn Arent, an assistant professor in the department of exercise science at Rutgers who led the study. “It’s not the greatest thing since sliced bread. It’s not magic. But for an elite athlete who has been training for a long time, even a 3, 4 or 5 percent increase in performance is a hard thing to come by.”

Similar research by Under Armour and Bite Tech with athletes at the Citadel, a military college, showed that using the mouth guards helped improve endurance and air flow.

Dena Garner, an assistant professor at the Citadel who has studied athletes using Bite Tech devices since 2005, said she thought some of her original findings were “a fluke.” But “every time I’ve done lactate studies with this mouthpiece, I’m finding there is a difference,” she said.

This year Captain Garner used an Under Armour mouthpiece while training for a marathon. Previously she “had been happy with running 10-minute miles,” she said, but wearing the mouthpiece, she consistently ran a mile in as little as 8 minutes. “It was pretty astounding to me,” she said. “I didn’t feel as tired as when I ran the 10-minute-per-mile pace.”

Clenching the teeth can lead to the release of the stress hormone cortisol, which, at excessive levels, can impede athletic performance. Having the Under Armour product in place “interrupts that flight-or-fight response,” said Bob Molhoek, chief executive of Bite Tech.

Chris McCormack, a professional triathlete who finished fourth in this year’s Ford Ironman World Championship, wore an Under Armour mouthpiece while he trained. “It stopped me from becoming tense,” he said, and “it doesn’t allow me to clench.”

Mr. McCormack said his running coaches have always told him to relax his jaw. “I never thought I’d hear myself say I need to wear a mouth guard in a running race, but I definitely see the value there,” he said.

So the dental nerd in me has me all excited over the new CEREC system we just acquired. Our old system was good, but this one is GREAT, as it allows me to make even more accurate restorations, faster and better, and best of all - completely onsite. Especially for cosmetic restorations which I previously sourced out to various labs.

So what does this mean to you? Well...in short, you can now spend less time in my chair with follow up visits, and more time at work :)

Performance Enhancing Mouthwear by Under Armour

If there's anything I DON'T like about my husband, it's the fact that he's REALLY big into martial arts. He trains, he teaches, and then he watches it again when he gets home - as if he hasn't had enough. Our DVR is filled with UFC reruns, and don't get me started on what we pay for pay-per-view subscriptions on fight night. One night while watching UFC, he actually came up with a pretty good idea about performance mouth guards, and wondered out loud if a properly fitted custom mouthpiece would indeed help with preventing knockouts. It was an interesting concept, and although I don't think a mouthpiece can keep your lights on after a Chuck Liddell right hook, based on what I know about Neuromuscular Dentistry, I'm 100% sure that a relaxed jaw can improve breathing and overall athletic performance.

Fast forward a few months, and Beth, our business manager introduced me to the Under Armour Performance Mouthwear. Beth is a HUGE football fan, and looked into this for her own son, who plays for a local high school. After looking into the technology and realizing it was similar to my own solutions for Neuromuscular Dentistry, I thought it would be a good cost effective fit for the weekend warriors that come into my practice.

I received the kit yesterday, and I'm very impressed!

So how does it actually work?

The custom mouthpiece is fitted in the office, and is designed to prevent clenching of the jaw and drops the jaw down and forward, creating a measured space between the teeth. Although the website states that relieving pressure on the TMJ changes signal cascades to the hypothalamus, I look at it from a physical perspective and less a chemical one. A properly placed jaw actually balances the entire body as it is connected to your Atlas axis, as well as the shoulder and back muscles. Maybe I'm simplifying it, but think of it like this.

Try running as fast as you can while clenching your jaw as hard as you can. Now, do the same exercise but with cotton rolls in your mouth to prevent clenching. How about trying the same exercise while throwing a punch? What you'll find is that when your jaw is relaxed and unclenched, you can run a bit faster, and punch a bit harder. This is because your jaw muscles are totally related to your entire body's balance.

Now for you folks that are suffering from severe TMJ disorders, ie. Migraines, neck pains, dizziness, etc. This is not the solution for you. Feel free to call me and I can go into additional details on how to treat those particular symptoms.

Hmmm...maybe I'll skip the morning coffee and donuts

Since moving to Sorrento Valley amongst all the technology and biotech companies, I've noticed one common denominator amongst some of my new patients...it's what I call "coffeedoughnut-itis". So I know that I completely made up that disease state, but I hate to break to you fans of the morning cup-o-joe and doughnut, just like your mom said, it's not good for you. I will admit that I am a fan of the doughnut, Stan's donut in the Santa Clara Bay Area being my favorite, however the quintessential professional in me must preach moderation to all you folks in a morning rush.

We understand how it works, you're late for work, skip breakfast and during the commute, pick up a doughnut and cup of coffee and off you go!

"This common quick-fix breakfast scenario can lengthen your time spent in the dental chair, reports the Academy of General Dentistry (AGD), an organization of general dentists dedicated to continuing dental education.

The sugars in doughnuts have been identified as a risk factor for gum inflammation and cavities. The AGD has reported findings that show the sugars in oatmeal cookies amount to only one-fifth of the sugars in plain doughnut particles.

The amount of sugar and cream in your coffee also can have a direct effect on the amount of cavity-causing bacteria. Tannins found in coffee etch into the pits and grooves of the tooth enamel, producing a rough, stained surface."

So this isn't anything new to your folks, so consider this posting a friendly public service announcement to brush often, floss like a champ, and try to wake up 30 minutes earlier for some healthy oatmeal and toast. If that doughnut and coffee can't be avoided - we're here if you need us!

Wine stained teeth?

You have all heard that red wine can stain your pearly whites, so the logic is that white wine would be much better right? MAYBE!

A study was done at my own alma mater, NYU College of Dentistry, in which two sets of cow teeth were each soaked in white wine and water for a period of one hour. The sets of teeth were then removed and soaked in black tea to study the staining effects after immersion in both liquids. The result was a noticeably darker stain on the teeth that were originally soaked in white wine versus the set that was soaked in water.

What does this tell us?

“Dipping teeth in white wine for one hour is similar to the effect of sipping the wine with dinner,” stated Mark Wolff, DDS, PhD, professor and chairman, who oversaw the study. “The acids in wine create rough spots and grooves that enable chemicals in other beverages that cause staining, such as coffee and tea, to penetrate deeper into the tooth.”

So all things equal, wine itself can cause changes in the tooth surface that would increase the likelihood of staining. Red wine, long known to stain teeth, contains a highly pigmented substance called "chromagen", thus adding to the staining effect.

So what to do?

Lucky for us, modern technology allows us options to cover up or reverse the staining process. Veneers and whitening is always an option, but daily maintenance with a whitening toothpaste should work just fine. So enjoy life a bit more and feel free to open up that $56,000 bottle of 1787 Chateau d'Yquem - we'll be here if you need to brighten your smile!

Sleep Apnea Linked to Memory Loss

I came across this article today discussing Sleep Apnea and it's link to memory loss. Although we've known for quite a while that there are many disorders linked to Sleep Apnea, I find this extremely interesting as a a number of patients I am treating have noted "forgetfulness" as a recent event. I originally thought that it was a simple case of "lack of sleep = a tired mind", but I stand corrected!

Sleep Apnea Linked to Memory Loss

Study finds disorder shrinks brain cells much the way alcohol, Alzheimer's do

WEDNESDAY, June 11 (HealthDay News) -- People with sleep apnea show tissue loss in brain regions that help store memory, a University of California, Los Angeles (UCLA) study shows.

"Our findings demonstrate that impaired breathing during sleep can lead to serious brain injury that disrupts memory and thinking," principal investigator Ronald Harper, a professor of neurobiology at the David Geffen School of Medicine at UCLA, said in a prepared statement.

People with sleep apnea stop breathing and awaken repeatedly during the night, leading to chronic daytime fatigue and memory and concentration problems. Research has linked sleep apnea to an increased risk of stroke, heart disease and diabetes.

In this study, the UCLA team used MRI to scan the brains of sleep apnea patients. The researchers focused on brain structures called mammillary bodies, located on the underside of the brain.

The study found that the mammillary bodies of the 43 sleep apnea patients were almost 20 percent smaller than those in 66 people without sleep apnea. The results will be published in the June 27 issue of Neuroscience Letters.

Repeated drops in oxygen experienced by sleep apnea patients may lead to brain injury, Harper suggested. He noted that lack of oxygen during an apnea episode can cause brain cell death.

"The reduced size of the mammillary bodies suggests that they've suffered a harmful event resulting in sizable cell loss. The fact that patients' memory problems continue despite treatment for their sleep disorder implies a long-lasting brain injury," Harper said.

"The findings are important, because patients suffering from memory loss from other syndromes, such as alcoholism or Alzheimer's disease, also show shrunken mammillary bodies," lead author Rajesh Kumar, an assistant researcher in neurobiology, said in a prepared statement.

"Physicians treat memory loss in alcoholic patients with massive amounts of thiamine, or vitamin B1. We suspect that the dose helps dying cells to recover, enabling the brain to use them again," Kumar said.

He and Harper plan to study whether taking supplemental vitamin B1 can help restore memory in sleep apnea patients. The vitamin moves glucose into cells, which prevents their death from oxygen starvation.

Sonicare and E. Coli?

New University Study Shows E. Coli, Mold and High Amounts of Other Bacteria Growing in Electric Toothbrushes; In Some Cases, Over 100 Times More Bacteria than their Manual Counterparts

Some of you may have recently heard the buzz about this study, where a University of Arizona study compared the bacterial contamination of manual vs. electric toothbrushes. The result was that electric toothbrushes contained 100 times the amount of bacteria than that of their manual counterpart.

Does this mean you should throw out your beloved Sonicare???? ABSOLUTELY NOT!

First and foremost, E. Coli for the most part is harmless, and is a part of our normal bacterial flora. In fact, E. Coli normally colonizes an infants gastrointestinal tract within 40 hours of birth! However, through the natural biological process of mutation, some strains can evolve into the nasty pathogenic strains that we see plastered all over the news.

For those of you who own Sonicare's, I'm sure you have all looked under the head to see that nasty goop that often builds up over time. That...my friends, is the result of oral bacteria colonizing in the dark damp crevices of your toothbrush! Now, don't panic. This doesn't mean that you're going to die from E. Coli contamination - just follow a few simple procedures to make sure your brush head is always clean.

1.) If the underside of the head is really slimy and dirty - switch it out.

2.) After each use, simply unscrew the head and rinse it under clean running water for 10-20 seconds.

3.) Dip it in Listerine or any other antiseptic mouthwash.

4.) If you've got a steam or UV sterilizer...use it.

5.) Shake it dry.

Norway Bans Dental Amalgam ("Silver") Filling

Happy New Year, everyone!

Here's food for thought as you begin your new 2008: For those people who still are looking to placing mercury amalgam ("silver") fillings in their mouths, think about as to why other countries are banning them.

According to American Dental Association:

"Norway has acted in the New Year to discontinue the use of dental amalgam. The action was part of a sweeping effort to restrict the use of mercury and was done so largely for environmental reasons. It's possible that Sweden may have taken a similar action, but we have not been able to adequately verify this with reliable information at this time (we will provide additional information next week if the issue flares up in the media). No new scientific studies or other new datea have been cited as calling for this sweeping action.

The ban is not expected to have any economic impact in these countries because of their national health care systems - for example, Sweden's health care system stopped paying for amalgam fillings years ago....."

A Patient's Experience with "Fixing the Bite"

I knew my jaw was lopsided ever since I was a kid. It had become
normal to me and I had forgotten about it. For years I had avoided
certain hard foods as well as opening my mouth wide. I started having
what seemed like random painful symptoms once I got my very first
crown. There was never a pain in my actual jaw, but elsewhere in my
face, and dentists used new crowns and root canals to try to treat the
problem. The pains continued, but there was no pattern. I couldn't
always describe them well or precisely pinpoint their location. I did
not know my jaw was shifting while it tried to compensate for a
misaligned bite. I broke teeth while chewing, and I had six root
canals, four retreatments, and several crown replacements.

I learned that with each new crown, my bite would shift and before
long, my chewing would be off, all over again. My dentists weren't
"getting it," so I would move on to another dentist hoping the next
one would know what to do. This is how my six crowns were each
replaced three times. I had a constant neck ache and had to crack it
several times each day. Insurance coverage was inadequate and I used
a family inheritance to pay some of the bills. I began arguing with
the professionals who were trying to help me.

The nightmare continued as I began seeing orthodontists and TMJ
specialists. I had three different splints over a period of time. I
found myself chewing on my right side in order to avoid pain on the
left. This lasted for a period of about three years and I did not
know that I was causing further damage to my bite. I also developed
nerve damage above certain crowns I had been pounding on while
chewing.

Then the debilitating sinus pain began and my confounded doctors and
family were wondering if I needed a psychiatrist. I found only
moderate relief from a headache specialist. I was the only one who
knew I wasn't crazy!

Each of a dozen dentists had offered me only partial solutions to my
problem. The only full solution offered to me was to go back to
orthodontics and couple it with jaw surgery, making a large incision
right through the bone. However, with my history of treatment
failures, finding a specialist willing to take on the job was an
issue. By this time, digesting my food had become a problem.

When I read a Dental Life insert in the Sunday newspaper, I knew I
had finally found the answer! Not to mention, the first day I walked
into Dental Design Spa, they treated me like a queen. I really felt
like a guest and I feel like one every time I go.

I am presently undergoing tensing treatment to relax the pressure
against my jaw. It is so gentle and painless that I have fallen
asleep in the dental chair. When I phone Dr. Agatep to report that my
jaw has changed, she lets me know this is a good thing. It means I am
getting closer to my ideal bite.

Finally, the doctor and her staff appreciate my need to question and
understand the details of my treatment. Although I have not had my
restorations done, I am looking forward to having a nice nap during
the procedure. I have certainly never felt so positive about a dental
experience.

Many People Think That "Periodontal Disease" Merely Involves the Mouth .... It's Not the Case!

ADA Seal of Acceptance Awarded to Chewing Gum

The American Dental Association (ADA) Council on Scientific Affairs (CSA) has awarded the Seal of Acceptance to Wrigley Orbit Sugar Free Gum, Wrigley Extra Sugar Free Gum and Wrigley Eclipse Sugar Free Gum, the first chewing gum products available to receive the Seal. CSA awarded the three Wrigley products the Seal after reviewing laboratory and clinical studies showing "that the physical action of chewing sugar-free gum for 20 minutes after eating stimulates saliva flow, which helps prevent cavities by reducing plaque acids and strengthening teeth."

Why Is A Dentist Treating Obstructive Sleep Apnea?

The first step in treating Obstructive Sleep Apnea (OSA) lies with proper diagnosis in the medical community. As is the case with all of our patients, OSA is diagnosed at a sleep lab, usually by a pulmonologist specializing in sleep medicine. Patients who are diagnosed are universally recommended for CPAP therapy. CPAP is an excellent treatment option that can eliminate nearly 100% of the apnea. The nasal mask fits on the head and is strapped in place all night forcing constant air pressure up the nose. While this is a highly effective treatment, numerous studies have shown that between 50% and 80% of patients prescribed CPAP simply cannot tolerate it and stop treatment altogether. This presents a serious, life threatening, problem for thousands of OSA sufferers.

In February, 2006 in the Journal SLEEP The American Academy of Sleep Medicine recommended that oral appliance therapy, provided by a dentist, be the first line for treatment of Mild/Moderate sleep apnea and an acceptable treatment for severe sleep apnea if CPAP failed. Oral appliances work by opening the bite and advancing the mandible. This forward position hold the tongue forward and prevents the tissues in the upper airway from collapsing and causing Apnea. Patients are highly compliant with oral appliance therapy because of the ease of use and immediate relief it provides.

The number of people suffering from OSA is staggering and huge majorities have failed or will fail CPAP therapy. Cutting edge technology has allowed those in the dental field to provide life saving therapy while working alongside our colleagues in the Pulmonology and ENT fields.

How many of you know what "Sleep Apnea" is? ... Dentists can now help!

In a definition taken from www.sleepapnea.org, the Greek word “apnea” literally means “without breath.” The three types of apnea are obstructive, central, and mixed. Of the three, obstructive is the most common. Despite the difference in the root cause of each type, in all three people with untreated sleep apnea stop breathing repeatedly during their sleep, sometimes hundreds of times during the night and often for a minute or longer.

Untreated, sleep apnea can cause high blood pressure and cardiovascular disease, memory problems, weight gain, impotency, and headaches. Moreover, untreated sleep apnea may be responsible for job impairment and motor vehicle crashes. Fortunately, sleep apnea can be diagnosed and treated. Several treatment options exist, and research into additional options continues.

Signs to look for are swollen tonsils or adenoids, allergies, inflamed sinus membranes, an anatomically small airway, or a combination. An oral airway obstruction occurs because of malocclusion, obesity (excess fat on the neck and chest), a hormonal imbalance/deficiency, pregnancy, an anatomically small airway, or combination. The sounds of snoring that a bed partner may hear are the result of pharyngeal tissues vibrating against one another. To sufficiently oxygenate the body, the air must force its way through the airway, partially or fully obstructed due to loss of patency. Gasping, choking or spasms and subsequent awakening are the body’s reaction to loss of oxygen and are indicative of a complete obstruction of the airway. Any obstruction of the airway is extremely detrimental to the intricate functioning of the cardiovascular, lymphatic, gastrointestinal, and nervous system.

Questions to ask include: Do you snore? Do you feel rested in the morning? Do you suffer from morning headaches? Do you have acid reflux? Do you have high blood pressure?

Great News!!! Dental Design Spa's Business Manager, Tina Compton, gave birth to a beautiful healty baby boy early this morning at 2:45 am! Weighing 6 lbs 9 oz and 19 inches long, we welcome baby William Gregory Compton!

This is a reminder that a new California law, Education Code Section 49452.8, now requires that your child have an oral health assessment (dental check-up) by May 31st in either kindergarten or first grade, whichever is his or her first year in public school. What Is Gum Disease?

Gum disease is know as gingivitis in the dental world. This is the first
stage of periodontal disease and is still manageable and even curable. It
occurs when the bacteria is housed in the natural space between the tooth
and gum without affecting the bone. However, gingivitis progresses to
periodontal disease if not addressed early enough. Once the bone begins to
disinigrate it is considered periodontal disease. These diseases are

One Day Crowns...

In the past if you've ever had a crown done you have probably started off with your first in a series of appointments to the dentist. The very first appointment being when the dentist prepares the tooth for a crown. After the dentist prepares the tooth for a crown the dentist then takes an impression to send to the lab for the crown to be fabricated. Two weeks later, and after wearing what is usually an uncomfortable temporary, the dentist then places the real crown.
Crowns can now be placed much faster by having the entire process completed in one day. This can only be done if the dentist is using a "CEREC" machine, which can allow the dentist to prepare the tooth for a crown, fabricate the crown, and cement the crown all in one day. Caring for your babies teeth

Here are some helpful hints that can start your child off with a healthy mouth.
-Before your baby has teeth, wipe there gums with a wet wash cloth after every feeding and before bed.
-Start brushing your child's teeth with a soft brush as soon as the first tooth appears with a very small amount of toothpaste, if your child is unable to spit - then wipe out the excess tooth paste.
-Your child's first visit to the dentist should be right around their first birthday.
-Never clean a dirty pacifier by placing in your own mouth, the bacteria that causes decay and periodontal disease is contagious.
-Once children are old enough to chew gum allow them to chew sugar free gum with xylitol as often as possible to prevent decay.

New Kindergarten Oral Health Requirement

What does the AB 1433 oral health requirement for children mean?

That we, as dental providers, have a wonderful opportunity before us to improve the oral health of children in California. The California Dental Association (CDA) successfully sponsored the landmark legislation this past year requiring children attending public school for the first time at either the kindergarten or first grade level to receive a dental check up. All children must receive a comprehensive examination, or at the very minimum an assessment, with their dental provider before the May 31 deadline. The purpose of this new law is to rejoin children to establishing a good source of regular dental check ups through what we call a good dental home. It as at these regular checkups, started at a young age, that a parent along with the child become aware and educated about the condition of their child’s oral health, the consequences if disease is not treated, and the benefits of ongoing care. Here at Dental Design Spa we feel it is very important for a child to establish trust and rapport with their dentist at an early age.

If your child already has a dental home with us and has had a dental examination within the last 12 months, the results of that exam will satisfy the requirement of the new law. Are Your TMJ (Temporomandibular Joints) Healthy?

The temporomandibular joints (the TMJ's) are two joints that are a part of the chew and swallowing functions. They are important in supporting the mandible to the skull by means of many muscles. Without two jaw joints our ability to open and close the mouth and move the jaw from side to side will be greatly diminished.

The TMJoints are often overlooked and not seriously considered, since on routine examinations the joints are hidden behind tissue. The oral cavity, the gums and teeth, are mostly seen since those are the parts that we see the easiest. Routine x-rays are taken of the teeth to check for cavities. Pocket probing measurements around the teeth and gums are recorded regularly to check for gum disease. But the jaw joints are rarely considered as important. Most dentist, physicians, chiropractors, osteopathic physicians, etc. are consider these two joints in their overall evaluation since most of the health professionals have had very little training in this area.

What the Reports Say..... and Their Findings.

The American Council for Headache Education in 2001 reported as many as 45 million Americans suffer from headaches. Tension type and migraine headaches are the most common, cluster headaches is the most rare. It has been reported that 85% of headaches will respond to treatment.

The Nuprin Pain Report by Louise Harris Associates reported,

* "Some feel that 70-80% of those with headaches have significant "muscles of mastication component" that are often undiagnosed and untreated.
* That compares with 15,100,000 workdays lost due to dental pain.
* Over 30% of all work days lost is reported in the study may have a dental component (including the muscles of mastication).

If Dentists were able to reduce the headache problems in just their area of responsibility we would save millions of lost work days and maybe reduce the usage of a billion dollars of pain meds.

In 2002 it was reported:

* Estimates indicate that there are more than 45 million headache sufferers in the United States.
* 58% of migraine attacks require some bed rest for a total of 112 million migraine-related bed ridden days per year for the US population.
* Migraine costs to employers is estimated at $13 million annually.
* The average migraineur loses 245 hours per year due to attacks, 84 hours coming from lost work time.

In a more recent Ocotber 2006 study, researchers have reported,

"People whose recurrent headaches have been diagnosed as tension-related actually may be suffering from temporomandibular muscle and joint disorder, or TMJD, a study headed by a researcher from the University at Buffalo's School of Dental Medicine has shown."

"Results showed that examiners could replicate tension-headache symptoms in 82 percent of subjects by performing the clinical examination of the temporalis muscle, which is involved in TMJD."

"The temporalis muscle is responsible for closing the jaw and is involved in chewing, but these core functions of that pair of muscles often are ignored when the presenting complaint is "headache," as opposed to jaw pain, Ohrbach said.

"Because headache is so incredibly common, it often is regarded as inevitable, and if sufferers label the pain as 'headache,' they may not seek help," he said. "Or if they do seek help, the label of 'headache' typically will propel the individual to a physician or neurologist for consultation.

Everybody knows the two primary things teeth are used for - eating and stopping hockey pucks. But what may surprise you is that your teeth can provide clues into your health like virtually no other outward part of your body. Why? The biggest concern when it comes to aging and your teeth isn't the presence of periodontal disease, which can make you up to 3.7 years older. Gum disease (gingivitis) has been linked to many other health problems, presumably because the same bacteria that cause periodontal disease can also trigger an immune response that causes inflammation and hardening of the arteries. That same plaque that causes tooth decay - that sticky coating of bacteria, saliva, and three -day-old cauliflower - can also contribute to the plaque in your arteries. The next time you open your mouth to have your teeth cleaned, your dentist might learn a whole lot more about your personal life than your conversation discloses.

Dentists are detectives of a sort, but they don't have to follow patients around to get answers about their lifestyle. It's all there between the lips.

Bleeding gums? The patient could be anemic. Eroded tooth enamel? Perhaps a sign of bingeing and purging. Wisdom teeth infected? Heart disease might be taking hold.

According to the American Dental Association and WebMD, diseases and health conditions that create changes in the mouth include:

Bulimia: Bingeing and purging stimulate the body to produce acid that destroys tooth enamel. Gum recession and lots of cavities also are indicators.

Diabetes: The giveaway is the telltale “sweet” acetone breath. Inflammation of the gums in people who brush and floss regularly could indicate impaired white blood cell production, a byproduct of diabetes. Abscesses also can occur.

Gastro esophageal reflux disease (GERD): causes the same symptoms as bulimia – eroded tooth enamel and multiple cavities.

Heart disease: Infection around the molars (pericoronitis), especially the wisdom teeth, is a strong predictor of heart disease, since the bacteria in the mouth and the plaque the bacteria causes also can appear as plaque in arteries.

Infection: A foul odor from the mouth often is a byproduct of infection in the mouth, lungs or stomach.

Leukemia: Gums often turn fiery red and are very inflamed and “spongy.”

Latest Update on Periodontal Disease and Pancreatic Cancer Risk in Men

According to a recent study, men with a history of periodontal disease could be at increased risk of developing pancreatic cancer. This finding, presented at the 2006 Frontiers in Cancer Prevention Research meeting in Boston, received online news coverage from Reuters Health Link opens in separate window. In this study, researchers from the Harvard School of Public Health, the University of San Juan, and the Dana-Farber Cancer Institute aimed to determine if periodontal disease or tooth loss may be related to pancreatic cancer. As noted by the research team, previous studies suggested a potential association between periodontal disease and pancreatic cancer, but those studies could not discount the role of confounding factors such as smoking history.

To analyze this periodontal-systemic association further, the authors reviewed 16 years of health data on over 51,000 male health professionals, all between the ages of 40 and 75 years at the study’s inception in 1986. The data were compiled as part of the Health Professional Follow-Up Study, an ongoing evaluation of health factors that may be related to cancer and other diseases.

Over a 16-year follow-up period (between 1986 and 2002), 216 men developed pancreatic cancer. After adjusting for age, smoking history, diabetes, obesity, diet and other potentially confounding variables, the reviewers found that men with a history of periodontal disease had a 63 percent increased risk of pancreatic cancer than men without a history of periodontal disease. While the prospective cohort study provides a long-term statistical association between periodontal disease and pancreatic cancer, it does not establish a definitive cause-effect relationship between the two conditions.

Pancreatic cancer is often detected at an advanced stage, and is commonly linked with tobacco smoking, age, or family history. While the authors claimed that periodontal disease or tooth loss may increase pancreatic cancer risk through “plausible biological mechanisms,” further research is required to validate this association. The role that diabetes played in the results also requires further investigation, because diabetes is associated with both periodontal disease and pancreatic cancer.

Today, over 70 percent of the adult U.S. population has some form of periodontal disease. To maintain oral health and prevent periodontal disease, the ADA promotes the importance of good oral hygiene and regular dental care. The ADA also encourages dentists to take thorough health histories, evaluate patients for oral diseases, promote tobacco cessation, and identify risk factors that may predispose patients to periodontal disease.

There is plenty to think about during pregnancy. For most women, maintaining good health is a primary goal. In order to achieve it, pregnant women are encouraged to eat healthier, avoid smoking and drinking, and incorporate moderate exercise into their day. But there needs to be another component to their daily health routine-superior oral care.

Many expectant mothers may simply believe that pregnancy leads to a deterioration of their oral health. But with a bit of extra attention, good oral health can be maintained all through the pregnancy. More importantly, maintaining healthy gums may be linked to delivering a healthy baby.

Many women experience more bleeding and swollen gums during pregnancy. These gum problems develop during the second trimester and are due to an increase in estrogen and progesterone that stimulates blood flow. The resulting changes are making it easier for plaque to develop on the teeth, which, as a result, will further enhance gingival inflammation and its clinical signs. However, a thorough oral care routine, as recommended by dental professionals, can help pregnant women keep their teeth free of plaque and offset the risk of gum disease.

An Oral Routine for Pregnancy

During pregnancy, daily brushing and flossing are important. Some women are very tired and may ignore the significance of their daily oral hygiene routines. Knowing how important oral hygiene is may provide the needed incentive. It has been reported that pregnant women can experience nausea during tooth brushing. If this is the case, using clinically proven anti-plaque and fluoride mouthwashes are recommended. And continue flossing, doing it more often to compensate.

Also, understand that eating frequently throughout the day, which many pregnant women find themselves doing to ease nausea, isn't in the best interest of your teeth, especially snacks made up of starchy, sticky or sugary snacks, such as sodas, crackers and hard candies. These foods generate more plaque than others and can lead to cavities. If you do need to snack, choose crunchy vegetables or fruit. They increase the flow of saliva, which will neutralize the acids plaque produced from snack including carbohydrates. Also, try to brush after snacks, or at least rinse the mouth with water.

Make regular dental visits a priority, just as you would an appointment with the obstetrician. When you go for dental care, be sure to inform your dentist and/or dental hygienist that you are pregnant. If gum disease develops, indicated by sore or inflamed gums, see a dentist immediately.

Remember, oral care is always important, but even more of a priority during pregnancy. Cultivate and maintain good habits and gum disease can be prevented, giving you one less thing to worry about during pregnancy.

Worried About Bad Breath? Here's a simple way to keep your breath clean and fresh!

In addition to brushing and flossing, a simple additional step, scraping the tongue, can make a difference! Your tongue has grooves and bumps that act as a home to bacteria that give off gases, which cause bad breath. Scraping the tongue can help reduce the number of those bacteria.

Articles in General Dentistry show studies have shown that tongue scrapers are effective at reducing halitosis, at least short-term.

With the holiday coming up, family and friends coming to visit ... if you're worried about controlling your bad breath, don't ignore the tongue.

Note that bad breath can also be a sign of a medical disorder, such as a respiratory tract infection, chronic sinusitis, diabetes, gastrointestinal disturbance, liver or kidney ailment.

If you have heart disease-or are at high-risk of developing heart problems-you need to be aware of the link between oral health and systemic health.

Recent studies have revealed that people with periodontal disease very often have heart disease as well. Researchers believe that this is due to oral bacteria present in gum disease, which can affect the heart if they enter the bloodstream. It has been shown that these bacteria can attach to fatty deposits in the coronary arteries and contribute to arterial clot formation. Such clots can dislodge and are responsible for heart attacks and strokes.

While periodontal disease doesn't necessarily cause heart disease, it is nevertheless a good risk indicator. In fact, people with severe gum disease have double the risk of stroke or heart attack. Fortunately, elimination of the gum infection by thorough professional care can eliminate the bulk of those bacteria and may actually lower the risk to the heart. Excellent plaque control through a good oral hygiene will decrease the risk for re-infection and ultimately may help enjoy better overall health long-term.

Remember, the mouth is the gateway to the body. Take care of your gums and teeth and it can pay off. Whether you see them or not, good oral care can have an impact on the health of other areas of your body-including your heart.

If you have, or are at risk of getting, heart disease:

* Keep your mouth healthy with twice daily brushing, using a high quality toothbrush and a fluoride toothpaste. For most people it is easier to remove plaque with a powerbrush, for example the Sonicare, than with a manual toothbrush. Floss daily as well.
* See your dentist and dental hygienist as directed and make sure they know about your heart condition and overall health.
* In turn, alert your physician if you have periodontal disease.
* If you have gums that are red or irritated or that bleed easily, see your dentist immediately.
* Use medications, including antibiotics, exactly as instructed.

What Antidepressants Can Do to Your Mouth!

If you and your doctor have determined that an antidepressant medication is the right choice for you, the side effect profile, I'm sure, have already been considered. We are aware of the fact that medications can have side effects. Of course, we are all different, and therefore we respond differently. Some individuals experience these effects and some do not.

So how does antidepressant medications affect the mouth?
One of the many side effects include DRY MOUTH or "XEROSTOMIA."
In a mouth's normal flora, saliva contain both the "good and bad bacteria." In a case of a dry mouth, without the "good bacteria," the mouth is not able to fight off the the "bad bacteria" that can cause cavity.

Simple ways to prevent dry mouth (thus preventing cavities):

2. Chew on sugarless gum or suck on sugarless hard candy

Researchers found no benefit for men using airway pressure therapy

MONDAY, Nov. 27 (HealthDay News) -- Despite researchers' hopes, prescription sleeping pills do not boost the effectiveness of therapy aimed at relieving obstructive sleep apnea.

Obstructive sleep apnea is a troubling condition marked by multiple nighttime awakenings. The gold-standard treatment is a therapy called "continuous positive airway pressure" (CPAP).

"CPAP treatment improves daytime alertness and quality of life for most patients with OSA and may prevent some of the long-term complications of this disorder, including heart attacks and strokes," study author Capt. David A. Bradshaw, of the Naval Medical Center, in San Diego, said in a prepared statement.

"Yet, many people find CPAP difficult to use. People with a good initial experience are more likely to use CPAP regularly. Our hypothesis was that a sleeping pill might help new CPAP users adjust to sleeping with the equipment and promote long-term usage," Bradshaw said.

In their four-week study, Bradshaw's group compared the effectiveness of CPAP among 72 newly-diagnosed male patients with obstructive sleep apnea.

They found that those who took sleeping pills were no more likely to use their CPAP machines than those who took a placebo.

Despite the study's findings, published in the November issue of Chest, Bradshaw said he and his colleagues believe that sleeping pills, when used correctly, may benefit certain patients with OSA.

"Studies have shown that almost half of patients with OSA have insomnia complaints. Our study does not support prescription sleeping pills for new CPAP users, but OSA patients with insomnia symptoms might benefit," he said.

Snoring ... It's NOT a Laughing Matter!

One of the most annoying sounds that you can experience is to hear someone else snoring. Snoring is that "log-sawing" noise that keeps everyone awake, from
efforts to force air through an airway that is not fully open. There are
many anatomical structures that can close our airway among them the soft palate, uvula, tonsils and adenoid tissue. Special folds in the nasal passages known as "tubinates" can also cause snoring when swollen. Swollen turbinates are the most common cause for snoring in adults. In children, snoring is mainly caused by swollen tonsils and adenoids. When a person snores they are mainly mouth-breathing. When you mouth-breathe, your blood is not properly oxygenated. This lack of oxygen saturation results in drowsiness the next morning, after a so called "full nights's sleep."
Since all of your body's cells require oxygen, the heart must beat harder which causes the right vertricle to enlarge. Over time, this can progress the seizures, heart attack and sudden death. Snoring is also the first indicator of the potentially life-threatening disorder called Obstructive Sleep Apnea. In this disorder the airway totally collapses, despite efforts to breathe.

It is obvious that snoring is nothig to laugh about. Since it has some rather serious ramifications associated with it, I screen all my patients for airway obstructions during exams.

Technology Helps Dentists Screen For Oral Cancer

According to the American Cancer Society, more than 30,000 new cases of oral cancer will be diagnosed in the U.S. this year. Local dentists are trying to get a jump on oral cancer with FDA approved technology that's getting a glowing recommendation.

"I like to know. I like to be on top of my health and my dental hygiene," said Brian Compton.

Which is why Compton says he worries about oral cancer, but he's thankful his dentist Doctor Kathrina Agatep at Dental Design Spa does, as well.

Nowadays, in 2006, it's not so much drilling and filling of a tooth anymore, it's simply looking at everything else," said Doctor Agatep.

That includes oral cancer which may be caused by tobacco use, alcohol and sun exposure, among other things.

Screenings for the cancer are part of a routine dental examination, because the earlier the cancer is detected, the better. But dentists like Doctor Agatep are taking the examination a step further.

"There are a lot of things that the naked eye can not see and with this ViziLite it enables us to see every little detail," continued Doctor Agatep.

To help detect abnormalities in a patient's mouth early on, Doctor Agatep uses the FDA approved technology called VizilLite.

"The test is actually very simple. It's a non-invasive, painless procedure," described Doctor Agatep

Patients first rinse with a cleaning solution for at least 30y seconds. Then, the overhead lighting is dimmed and Doctor Agatep uses a specially designed light technology to look for any problems.

"By using this light, anything abnormal will actually come out to be, will come out to be illuminated," Doctor Agatep said.

Today, Compton gets the good news that no abnormalities were revealed. He says ViziLite is a simple test that brings him great comfort.

It's done quickly, painlessly and you're in and out and you have that peace of mind," Compton noted.

ViziLite may not be covered by all insurance companies, so talk to your dentist ahead of time.

During the general exam Dr Agatep "found" infections under all five silver fillings, even though different So.Cal. dentists did them less than 6 yrs ago. (The average probability of infection under a silver filling is 1% in 5 yrs. A second opinion one month later did not confirm the presence of infection anywhere.) So she recommended onlays, but later they got disappointed with some restrictions of my HMO insurance and refused to accept it. They don't do any silver fillings at all: probably too cheap for them. The dental service that I eventually requested and received was excellent, but what Dr Agatep told me about the health of my teeth was quite misleading. Also check the dates of the previous reveiws: don't you notice something funny there?

My Response

Dear Patient,
First and foremost I want to apologize that our treatment was not up to
your expectations. The most important thing is the overall health of
your teeth, so I’m glad that you were able to find a provider that would
offer amalgam fillings.

I wanted to personally make a few comments just so that there is an understanding with your case.

In dentistry, the “infection” of a tooth is usually treated with
antibiotics and a possible root canal; NOT an onlay. So I would never
recommend treatment of an infection with just an onlay. However, a fractured amalgam filling (whatever the cause may be) is usually treated with a replacement, either with a composite (if the old restoration is small) OR an onlay if the old restoration is big, or a crown in extreme cases.

With regards to amalgam fillings, we choose not to provide these as a
treatment option in response to the numerous studies surrounding mercury
based fillings.

The following references will clarify our decision to avoid using
amalgam fillings as an option for treatment.
